Dr. Arun Kalyanasundaram is one of India’s foremost interventional cardiologists – a career built across two countries, three decades, and more than 2,500 of the most complex coronary procedures performed anywhere in the world. He specialises in Chronic Total Occlusion PCI, Complex High-Risk Interventions (CHIP), and advanced structural and peripheral vascular interventions, bringing to India a standard of care forged at the Cleveland Clinic and refined across cath labs from Seattle to Singapore.

He is the Asia-Pacific CTO Club Director for India, the physician who sets the guidelines for complex coronary intervention across South and Southeast Asia, and to whom the most difficult and failed coronary cases across the subcontinent are referred. Among the small global group of operators who have independently performed more than 1,000 CTO PCI procedures, he is one of the very few based in Asia.

From Madras Medical College to Cleveland Clinic, Swedish Medical Center Seattle to the founding of India's first dedicated CTO PCI programme - a path defined by depth, not breadth.

1993

Foundation in medicine at one of Asia’s premier and oldest institutions. Graduating in 1999, the discipline and patient-first philosophy of Indian medical training shaped every complex interventional cardiology case that followed.

1999

Rigorous grounding in evidence-based medicine, epidemiology, and clinical research design. This foundation directly informs his approach to evaluating every procedure against clinical evidence and underpins his publication record of 30+ peer-reviewed papers.

2002

Seven formative years from resident to Chief Fellow in cardiology. Obtained independent funding as Principal Investigator for a cardiac imaging study. Comprehensive training in invasive haemodynamics and catheterisation laboratory procedures.

2009

Cleveland Clinic – ranked the No. 1 hospital for heart care in the United States for 30 consecutive years (1995–2024) by U.S. News & World Report. Two consecutive fellowships: complex coronary procedures including CTO PCI (2009–2010), followed by structural heart disease and peripheral vascular interventions (2010–2011). Physicians who emerge from this programme represent a small global cohort. Fewer still return to practise full-time in South Asia.

The K14 Technique.

The K14 Stingray CART technique is a novel CTO PCI innovation developed and published by Dr. Arun Kalyanasundaram in Catheterization and Cardiovascular Interventions in 2023 (PMID: 36617386). It is a modification of the standard antegrade dissection re-entry (ADR) approach that solves a previously unsolved failure mode in the Stingray re-entry step — when the wire does not adequately penetrate the Stingray port into the true lumen. Dr. Arun’s modification uses a CART manoeuvre to overcome this resistance, allowing successful re-entry in cases that would otherwise fail.

K14 stands for Kalyanasundaram – his nickname from fellowship days at the Cleveland Clinic. The technique has been adopted internationally. This is one of the clearest examples of a physician who does not merely perform complex interventions but advances the field itself.
